GLP-1 pens and vials decoded: clicks, units and dose math for every device

The one rule behind all of it

A dose in milligrams equals the volume injected times the concentration: milligrams = millilitres × mg/mL. The labels give each device's concentration, so every volume below can be checked with that one line. The danger is not the arithmetic but assuming two devices of the same drug hold the same concentration. They often do not. Everything on this page is from the current US labels as of October 2, 2026, and is information, not dosing advice.

Zepbound and Mounjaro (tirzepatide)

Tirzepatide High is sold as Zepbound for weight management and Mounjaro for type 2 diabetes, and both labels list the same four presentations 1 2:

The trap is between the two vials. A single-dose 2.5 mg vial holds 2.5 mg in 0.5 mL, or 5 mg/mL. A multi-dose vial for 2.5 mg doses holds 10 mg in 2.4 mL, or 4.17 mg/mL, so each 2.5 mg dose is 0.6 mL 1. Drawing 0.5 mL from the multi-dose vial out of habit would give about 2.1 mg; drawing 0.6 mL from a single-dose vial is not possible, because it only holds 0.5 mL. On a U-100 syringe, the multi-dose volume is the 60-unit mark and the single-dose volume the 50-unit mark.

The labels start at 2.5 mg once a week for four weeks and increase in 2.5 mg steps after at least four weeks until the maintenance dose is reached 1.

Wegovy and Ozempic (semaglutide)

Two points stand out. Wegovy's FlexTouch and its 2.4 mg single-dose pen give the same 2.4 mg in the same 0.75 mL, both at 3.2 mg/mL; the FlexTouch simply holds four of those doses in a 3 mL reservoir, and since it comes only in 2.4 mg doses it covers the usual maintenance dose, not the escalation steps. And the Ozempic 2 mg/3 mL pen is labelled for four doses of 0.25 mg and two of 0.5 mg, or four doses of 0.5 mg; the pen sets those doses 4. Wegovy also comes as tablets of 1.5, 4, 9 and 25 mg, which involve no injection math at all 3. The Wegovy label titrates every four weeks to a usual maintenance dose of 2.4 mg once a week 3.

Clicks, partial doses and units

People sometimes try to get extra or smaller doses out of a pen by counting the clicks of its dial. None of the four labels describes doing this: each lists the pen's doses as fixed strengths, and the multi-dose pens as delivering a set number of set doses 1 3 4. A dose between the labelled strengths is outside the label, and the pen was not designed or tested to deliver it accurately.

Units cause a similar confusion. Insulin syringes are marked in units because insulin is sold at 100 units per mL; on such a U-100 syringe, every 10 units is 0.1 mL. For a GLP-1 vial, the units are only a reading of volume: 50 units is 0.5 mL, whatever is in it. Turning that into milligrams needs the vial's own concentration from its label, which is why a unit count copied from someone else's vial, or from a compounded product, can be badly wrong.

Before changing anything

Switching between a pen and a vial, or between single-dose and multi-dose vials, changes the volume needed for the same dose. The labels tell prescribers to make sure patients are trained on the specific presentation they receive and retrained if it changes 1 3. How many units is 2.5 mg of Zepbound from a vial?

It depends on the vial. A single-dose 2.5 mg vial holds 0.5 mL, the 50-unit mark on a U-100 syringe; the multi-dose vial for 2.5 mg doses is 4.17 mg/mL, so each dose is 0.6 mL, the 60-unit mark.

Can you count clicks on a Wegovy or Ozempic pen to get a different dose?

None of the labels describes it. Each pen delivers its labelled set doses, and a dose between them is outside the label and not something the pen was tested to deliver accurately.

How many doses are in a Zepbound KwikPen or a Wegovy FlexTouch?

Four. The Zepbound and Mounjaro KwikPen holds four set doses in 2.4 mL, and the Wegovy FlexTouch holds four 2.4 mg doses in 3 mL.

Are the volumes the same between a single-dose and multi-dose vial?

No. For tirzepatide, a single-dose vial dose is 0.5 mL and a multi-dose vial dose is 0.6 mL, because the concentrations differ.
