Thymalin: a Russian thymus preparation
Thymalin is approved in Russia; it is not approved in the US, where Drugs@FDA holds no application 1. Its most striking report concerns severe COVID-19: among middle-aged and elderly patients, hospital mortality was 40.9% with standard care, 28.4% with tocilizumab and 20.6% with thymalin, and thymalin doubled lymphocyte and monocyte counts (human study) 2. The groups were compared as treated, and a report like this needs a randomised trial before its mortality figures can be trusted.
Its other human studies are small and uncontrolled: in 15 people with early parkinsonism the authors reported improvement without a control group (human study) 3, and in 36 psychiatric patients with immune abnormalities they reported better responses when it was added to drugs (human study) 4. In rats with transplanted sarcoma it slowed or reversed tumour growth (animal study) 5.
Epitalon: cells and mice
Epitalon is a synthetic tetrapeptide (AEDG). In cultured human fetal fibroblasts it induced telomerase and telomere elongation (laboratory study) 6. In 54 female mice given monthly courses for life, average lifespan did not change (animal study) 7. The long human study often cited for it used epithalamin, a pineal extract (human study) 8. What they share
Both come from a research programme that has produced many peptides and reports, mostly published by the same group and often in Russian journals. That concentration is the main reason both carry low evidence grades in our encyclopedia: positive results that have not been reproduced by independent groups, in controlled trials, are not enough to judge benefit or safety. Thymalin's Russian approval reflects that country's regulator, not a trial standard recognised by FDA or EMA.

- Was it published by the developing group, or reproduced independently?
- Is the product the Russian pharmaceutical or a research powder sold under the same name?
- For epitalon, does the claim concern synthetic epitalon or the pineal extract 8?
- Am I subject to anti-doping testing 1?
How the differences add up (as of October 3, 2026)
Thymalin has an approval in Russia and a handful of human reports, the most notable uncontrolled; epitalon has cell results and a null mouse lifespan finding. Neither is approved in the US, EU, UK, Canada or Australia. For athletes, epitalon falls under S0 by inference, while thymalin's status is unsettled because of its Russian approval 1.
