Where each acts
The reproductive axis runs from the hypothalamus, which releases GnRH in pulses, to the pituitary, which responds with LH and FSH, to the ovaries or testes. Gonadorelin is synthetic GnRH, acting directly on the pituitary 1. Kisspeptin acts higher up, prompting the hypothalamus to release the body's own GnRH; in a study of men, kisspeptin-54 produced a peak LH of 12.5 IU/L in healthy men but only 0.4 in men with congenital hypogonadotropic hypogonadism, whose GnRH system does not work, separating the groups better than a GnRH test did (human study) 2.
That difference matters clinically. Gonadorelin can replace missing GnRH; kisspeptin needs a working GnRH system to have any effect.
Gonadorelin: decades of pump treatment
Two gonadorelin products were FDA-approved, Factrel in 1982 and Lutrepulse in 1989, and both are listed as discontinued in Drugs@FDA 1. Its main use was pulsatile treatment by pump. In women with hypothalamic amenorrhea, intravenous pulses induced ovulation in 91% and 96% of two groups, with line complications in up to 11% (human study) 3; in a series from 1984 to 1993, 75% of cycles ovulated and 18% produced a pregnancy, with few multiple pregnancies (human study) 4. In men with hypogonadism, a 2024 study using a pump every 90 minutes found sperm appeared in 73.3% over about 19 months (human study) 5.
Reactions are rare but documented: a woman on a gonadorelin pump developed skin reactions with a positive skin-prick test to the hormone (case report) 6, and an eight-year-old girl lost consciousness and had seizures within minutes of an intravenous gonadorelin stimulation test, recovering within 30 minutes (case report) 7.
Kisspeptin: an investigational IVF trigger
Kisspeptin's most striking results are in IVF, where a final injection triggers egg maturation. In a trial of 60 women at high risk of ovarian hyperstimulation, a single kisspeptin-54 injection matured eggs in 95%, with a 45% live-birth rate per transfer and no moderate, severe or critical hyperstimulation (randomised trial) 8. An earlier study found mature eggs at every dose tested (human study) 9. Twice-daily injections in the menstrual cycle did not disrupt cycles (human study) 10. How they compare
Gonadorelin is the older tool: it replaces GnRH, works even when the hypothalamus fails, and needs pulsed delivery to mimic nature. Kisspeptin is the newer idea: it works only through a functioning hypothalamus, which may make its stimulation more physiological, and its IVF results suggest a lower overstimulation risk in the women studied 8. Gonadorelin's approved products have been discontinued in the US 1, while kisspeptin has not completed the trials needed for approval. Neither is a do-it-yourself fertility or testosterone treatment.

- Is pulsatile GnRH available where I live, given the US products were discontinued?
- Is a kisspeptin trial recruiting that I might join?
- Am I subject to anti-doping rules? Both are prohibited.
How the differences add up (as of October 3, 2026)
Gonadorelin has decades of pump use, two discontinued US approvals and a small record of rare reactions. Kisspeptin is investigational, with encouraging IVF data and a diagnostic role in hypogonadism. Both are on the WADA list under S2.2.1, because they raise LH and testosterone 1.
