Palopegteriparatide is an FDA-approved prescription medicine, sold as Yorvipath, for hypoparathyroidism in adults. It releases parathyroid hormone steadily from one injection a day.
The label starts at 18 mcg once daily under the skin and adjusts in 3 mcg steps to keep blood calcium normal. The labeled range is 6 to 30 mcg; 30 mcg is the maximum.
Each dose is one injection. Blood calcium is measured 7 to 10 days after the first dose and after every change.

| Label: hypoparathyroidism in adults (calcium at least 7.8 mg/dL and normal vitamin D before starting) Approved labelYORVIPATH (palopegteriparatide) injection Prescribing Information, sections 1, 2, 5, 6 and 12 (DailyMed version 9, effective August 12, 2026) | Start 18 µg; adjust in 3 µg steps to a maximum of 30 µg | Once daily, as a single injection | Ongoing; calcium checked every 4 to 6 weeks once stable | Subcutaneous |
The dose is individual. The goal the label sets is blood calcium in the normal range without active vitamin D (such as calcitriol) or therapeutic calcium doses, meaning elemental calcium above 600 mg a day. Calcium to meet ordinary dietary needs may continue.
How the dose is adjusted
- Steps: up or down by 3 mcg.
- Pace: not increased more often than every 7 days, and not decreased more often than every 3 days.
- Checks: serum calcium 7 to 10 days after the first dose and after any change to Yorvipath, active vitamin D or calcium. Once the maintenance dose is reached, at least every 4 to 6 weeks; some people need further adjustment over time.
- Very high calcium: at an albumin-corrected calcium of 12 mg/dL or more, the label withholds Yorvipath for 2 to 3 days and rechecks.
- At the maximum: if calcium stays low on 30 mcg, the label points to adding or restarting calcium and active vitamin D, or other treatment.
Before the first dose
Within two weeks before starting, the label asks for a normal 25(OH) vitamin D level and an albumin-corrected serum calcium of at least 7.8 mg/dL. On the day of starting or stepping up, active vitamin D and calcium supplements are reduced according to the label's table, which depends on the calcium level and current calcitriol dose.
One injection, every time
The label is explicit: use only one injection to reach the daily dose. Two injections to make up one dose raise the risk of unintended swings in blood calcium, both low and high. The pen is injected into the abdomen or the front of the thigh, rotating the site daily. Early doses are given sitting or lying down, because the drug can lower blood pressure on standing. Pens are kept refrigerated at 2 to 8°C until first use.
Missed doses and breaks
Missed by less than 12 hours: take it as soon as possible. Missed by more than 12 hours: skip it and take the next dose as scheduled. After a break of 3 days or more, the label asks for a check for low calcium, may restart or increase calcium and active vitamin D, resumes Yorvipath at the previous dose, and measures calcium again within 7 to 10 days.
Frequently asked questions
What is the starting dose of Yorvipath?
18 mcg once daily, injected under the skin. It is then adjusted in 3 mcg steps according to blood calcium.
What is the maximum dose of palopegteriparatide?
30 mcg once daily. The labeled range is 6 to 30 mcg.
Can two injections be used to make one dose?
No. The label says to use only one injection for the daily dose, because two injections increase the risk of unintended changes in blood calcium.
What if I miss a Yorvipath dose?
If less than 12 hours late, take it as soon as possible; if more than 12 hours, skip it and take the next dose as scheduled. After 3 or more days off, calcium is checked.
