Ozempic, Wegovy, Rybelsus and the Wegovy pill: one molecule, four products

The same molecule, different products
Semaglutide is a GLP-1 receptor agonist, a modified version of the gut hormone GLP-1 that lasts long enough for weekly injection, or daily tablets with an absorption enhancer. Novo Nordisk sells it under three brand names in the US, and two of them now come in both injection and tablet form. The differences between the products are not cosmetic: each label sets out a different approved use, a different dose range and, for the tablets, a different way of taking it 1 2 3. Ozempic injection: for type 2 diabetes
The Ozempic injection label lists three uses, all in adults with type 2 diabetes: to improve blood sugar control alongside diet and exercise; to reduce the risk of major cardiovascular events in those with established cardiovascular disease; and to reduce the risk of sustained kidney function decline, end-stage kidney disease and cardiovascular death in those with chronic kidney disease 1. Weight loss is not a labelled indication, although it happens, which is why Ozempic is so widely prescribed off-label.
It is injected once weekly, at any time of day, under the skin of the abdomen, thigh or upper arm. The label starts at 0.25 mg weekly for four weeks, then 0.5 mg, with steps to 1 mg and then 2 mg if more glucose control is needed, each after at least four weeks; for the kidney indication, the target is 1 mg. A missed dose can be taken within five days 1.
Rybelsus and Ozempic tablets: daily pills for diabetes
Rybelsus is the original oral semaglutide brand. Its current label is shared with a newer product, Ozempic tablets, and covers both. They are approved for blood sugar control in adults with type 2 diabetes and to reduce the risk of major cardiovascular events in those at high risk 2.
The way they are taken matters more than with any injection. Both are taken once daily on an empty stomach in the morning with no more than 4 ounces of plain water, swallowed whole, and the label says to wait at least 30 minutes before eating, drinking or taking other oral medicines 2. Rybelsus starts at 3 mg daily for 30 days, a dose the label says is not effective for glucose control, then 7 mg, with an increase to 14 mg if needed. Ozempic tablets start at 1.5 mg, then 4 mg, with an increase to 9 mg if needed 2. The label states plainly that Rybelsus and Ozempic tablets are not substitutable on a milligram-to-milligram basis, and gives separate instructions for switching between them and from Ozempic injections 2.
Wegovy injection and tablets: for weight management and more
Wegovy is the semaglutide brand approved for weight management. The injection's label lists, alongside a reduced-calorie diet and more physical activity: reducing the risk of major cardiovascular events in adults with established cardiovascular disease and obesity or overweight; reducing excess weight and maintaining the loss long term in adults and children aged 12 and over with obesity, and in adults with overweight and at least one weight-related condition; and treating non-cirrhotic MASH, the liver disease formerly called NASH 3.
The injection starts at 0.25 mg weekly for four weeks and steps up every four weeks; the usual maintenance dose is 2.4 mg weekly, with the label giving indication-specific maintenance doses 3. Wegovy tablets, the 'Wegovy pill', are approved for adults for cardiovascular risk reduction and weight management. They start at 1.5 mg once daily for 30 days and step up every 30 days to a maintenance dose of 25 mg daily, with the same empty-stomach, water-only, 30-minute rules as the diabetes tablets 3.
Wegovy's label also carries a limitation of use: combining Wegovy tablets or injection with other semaglutide-containing products, or with any other GLP-1 receptor agonist, is not recommended 3.
Why tablet doses are so much bigger
Swallowed peptides are mostly digested before they reach the blood, so semaglutide tablets contain an absorption enhancer, SNAC, and are absorbed mainly in the stomach, according to the Rybelsus and Ozempic tablets label. Even so, the label estimates absolute bioavailability, the share of the dose that reaches the bloodstream, at only about 0.4% to 1% for Rybelsus 3, 7 and 14 mg, and about 1% to 2% for Ozempic tablets 1.5, 4 and 9 mg 2. That difference between the two tablet formulations is why the label says they cannot be swapped milligram for milligram: 9 mg of one is not equivalent to 9 mg of the other.
It also explains the strict routine. Food, other drinks and other pills in the stomach interfere with absorption, which is why the label asks for an empty stomach, no more than 4 ounces of plain water and a 30-minute wait 2. The Wegovy label adds a caution for people with type 2 diabetes taking Wegovy tablets: average semaglutide levels were lower than with the injection and more variable, so some may not reach effective levels, and the label suggests considering alternatives for those with an inadequate response 3. A tablet is convenient, but it is not a weaker or stronger version of the pen; it is a different delivery system with its own dose scale.
Why you can't just swap them
Because they share a molecule, it is tempting to treat these products as interchangeable. The labels say otherwise in three ways. First, doses differ widely between forms: oral semaglutide is absorbed far less efficiently than injected, which is why tablet doses run to 9, 14 or 25 mg daily while injections top out at a few milligrams weekly. Second, even between tablets, the label says Rybelsus and Ozempic tablets are not substitutable milligram for milligram 2. Third, each product is approved for different uses, which matters for insurance coverage and for which trials support the dose being used 1 3.
Switching between products is a prescribing decision with its own label instructions, not something to work out from the milligrams on the box. The same applies at the pharmacy counter: the labels treat each product separately, with its own indications and dose scale, so one semaglutide brand is not a stand-in for another.
Self-pay prices, as of October 3, 2026
List prices are high, but Novo Nordisk sells several of these products directly to self-paying patients through its NovoCare Pharmacy. Its Wegovy price guide, as indexed on October 3, 2026, listed Wegovy pens at $349 a month for doses from 0.25 mg to 2.4 mg, with new patients paying $199 a month for their first two fills of the 0.25 mg or 0.5 mg doses through December 31, 2026; a 7.2 mg Wegovy HD pen at $399 a month; and Wegovy tablets from $149 a month for 1.5 mg to $299 a month for the 25 mg maintenance dose 4. Prices change often, and insured patients may pay very different amounts, so check the current figure before deciding.

Which one fits, in plain terms
- Type 2 diabetes, preferring a weekly injection: Ozempic injection is the labelled option 1.
- Type 2 diabetes, preferring a daily pill and able to follow the empty-stomach routine: Rybelsus or Ozempic tablets 2.
- Weight management, cardiovascular risk with obesity, or MASH: Wegovy injection; for adults wanting a pill for weight or cardiovascular risk, Wegovy tablets 3.
- Already on one semaglutide product: do not add another, and do not combine with other GLP-1 drugs 3.

Teenagers: only one of the products
Of all these products, only Wegovy injection is approved for anyone under 18: its weight-management indication covers children aged 12 and over with obesity, while Wegovy tablets are approved for adults 3. The label notes that patients aged 12 and older treated with the injection had higher rates of gallstones, gallbladder inflammation, low blood pressure, rash and hives than adults, and that while hypoglycaemia was more frequent in adults with type 2 diabetes and obesity on Wegovy, there are insufficient data to say whether the risk is higher in adolescents with type 2 diabetes; it asks prescribers to teach young patients the signs of low blood sugar 3. For a teenager, then, the choice is narrower than for an adult, and the follow-up matters as much as the prescription.
Shared safety points
Because the active ingredient is the same, the core safety information overlaps: stomach side effects, especially while the dose is rising, and warnings about pancreatitis, gallbladder disease and low blood sugar when combined with insulin or sulfonylureas. Wegovy's label, for example, warns that combining it with insulin or an insulin secretagogue may increase the risk of hypoglycaemia, including severe hypoglycaemia, and that those doses may need reducing 3. - Which semaglutide product matches my diagnosis, and is it covered by my insurance for that use?
- Would a tablet suit my morning routine, given the empty-stomach and 30-minute rules?
- If I am switching products, what does the label say about the new starting dose?
- Do I take insulin or a sulfonylurea that may need adjusting?
The bottom line
Ozempic, Rybelsus and Wegovy are one molecule packaged for different jobs. Ozempic and Rybelsus are diabetes drugs; Wegovy is the weight-management, cardiovascular and MASH brand. The tablets need a strict morning routine and are not interchangeable milligram for milligram, even with each other. Choose by diagnosis and form with a prescriber, follow the label's dose steps, and never combine semaglutide products.
