Icotrokinra is an FDA-approved prescription medicine, sold as Icotyde, for moderate-to-severe plaque psoriasis in adults and in children aged 12 and older who weigh at least 40 kg. It is a small engineered peptide that blocks the IL-23 receptor, swallowed as a tablet rather than injected.
The label gives one dose: 200 mg by mouth once a day. There is no starting dose and no step-up. The tablet is taken on waking, on an empty stomach, with water, and food waits at least 30 minutes.
This page summarizes the label for education. A prescriber decides whether it suits you and checks for tuberculosis and vaccinations before starting.

| Label: moderate-to-severe plaque psoriasis, adults and children 12+ weighing at least 40 kg Approved labelICOTYDE (icotrokinra) tablets Prescribing Information, sections 1, 2, 5, 6 and 12 (DailyMed version 2, effective March 17, 2026) | 200 mg | Once daily, on waking, 30 minutes before food | Ongoing | Oral |
The label sets the same dose for everyone it covers. The weight floor of 40 kg applies to children aged 12 and older; adults have none. Because there is only one dose, nothing on the label is adjusted up or down for response the way GLP-1 medicines are stepped.
How to take it
- When: on waking, once a day.
- Stomach: empty, with water. Wait at least 30 minutes after the tablet before eating. The entry explains why: a high-fat meal cut exposure by 43%, and only a small fraction of an oral peptide is absorbed even fasting.
- Swallow it whole. The label says not to crush, split or chew it.
- If swallowing tablets is hard: put one tablet in a cup with at least 120 mL (4 ounces) of water and let it disperse for a few minutes. It may not dissolve completely and can look yellow, milky or cloudy; the small pieces are safe to swallow. Swirl and drink it all, add at least another 120 mL of water to the cup and drink that too, so the whole dose is taken. Finish within 15 minutes of dispersing it.
Missed dose
Per the label, take the missed dose as soon as possible, then go back to the normal schedule the following day. There is no instruction to double up.
Before the first dose
The label asks the prescriber to consider testing for tuberculosis before starting, based on clinical judgement, and to bring vaccinations up to date first. Live vaccines are avoided during treatment. An important active infection is treated before Icotyde is started.
In the pooled trials, adverse event rates through week 16 were comparable to placebo; infections are the label's main caution. The label lists no contraindications. For the evidence behind the drug, see the icotrokinra entry.
Frequently asked questions
What is the recommended dose of Icotyde?
One 200 mg tablet by mouth once a day, taken on waking on an empty stomach with water, at least 30 minutes before food. The label has no other dose and no titration.
What should I do if I miss a dose of Icotyde?
The label says to take the missed dose as soon as possible and return to the normal schedule the following day.
Can Icotyde be crushed or split?
No. The label says to swallow it whole. For people who cannot swallow tablets, it can be dispersed in at least 120 mL of water and drunk within 15 minutes, followed by a second rinse of the cup.
Why does it have to be taken before food?
It is a peptide taken by mouth and little of it is absorbed. A high-fat meal cut exposure by 43%, so the label asks for an empty stomach and a 30-minute wait before eating.
