LSALT peptide

also known as: Metablok, LSALT

A 16-amino-acid peptide, given by drip, that blocks the neutrophil docking receptor DPEP1 in the lung, liver and kidney. It was tolerated but showed no clear benefit in a 61-patient COVID-19 trial, and a 240-patient trial of kidney protection in heart surgery is recruiting.

LSALT peptide (Metablok) is a 16-amino-acid peptide infused into a vein to block dipeptidase-1 (DPEP1), the receptor neutrophils use to stick to blood vessels in the lung, liver and kidney. Pretreating mice with it before the kidney's blood supply was cut off and restored reduced neutrophil adhesion and left less kidney injury (PMID 35108057). In 61 patients hospitalised with moderate-to-severe COVID-19, 93.3% of those given it against 90.3% on placebo were alive and free of respiratory failure and dialysis at 28 days, a trial far too small to test that difference (PMID 38490660). A 240-patient phase 2 trial in on-pump cardiac surgery is recruiting (NCT05879432). It is approved nowhere.

Used in human trials: 5 mg, once daily, intravenous infusion (Studied dose) Details ↓

Mechanism of action

Dipeptidase-1 (DPEP1) sits on the lining of blood vessels in the lung, liver and kidney, where it works as a physical docking point for neutrophils rather than through the enzyme activity its name describes; the peptide binds it and blocks the docking, leaving the enzyme alone (PMIDs 31442408, 35108057). In mice, pretreatment before the kidney lost and regained its blood supply cut neutrophil adhesion in the peritubular capillaries and the recruitment of inflammatory monocytes, and left less kidney injury; blocking CD44 added to the effect and blocking ICAM-1 did not (PMID 35108057). In the COVID-19 trial the treated group's blood CXCL10, a signal that draws immune cells into tissue, fell further from baseline than on placebo (PMID 38490660).

Primary uses

  • Investigational: preventing acute kidney injury after on-pump cardiac surgery (phase 2, recruiting)
  • Investigational: preventing respiratory failure and acute kidney injury in hospitalised COVID-19 (phase 2a, completed, no clear benefit)

Typical dosing

Human studyPMID 38490660 5 mg once daily (intravenous infusion), phase 2a

Used in human trials: 5 mg infused over two hours once daily for up to 14 days in the phase 2a COVID-19 trial (PMID 38490660). The recruiting cardiac-surgery trial gives 10 mg over one hour twice daily for five days, the first dose at least an hour before anaesthesia (NCT05879432), and the phase 1 study gave single doses from 0.01 to 5 mg and 1 to 5 mg once daily for three days (NCT03772678). Hospital infusions given inside trials, not recommendations, and no trial has shown that any of them changes an outcome.

Regulatory status

Not approved anywhere: Drugs@FDA and the openFDA label endpoint hold no record for LSALT peptide or Metablok, and the EMA medicines list holds none (searched November 4, 2026). Arch Biopartners has registered three trials of its own: a phase 1 single and multiple ascending dose study in 52 healthy adults in Australia (NCT03772678, completed March 2020, results posted), the phase 2a COVID-19 trial in 61 patients (NCT04402957, completed, published as PMID 38490660) and a 240-patient phase 2 trial of kidney protection in on-pump cardiac surgery (NCT05879432, recruiting, primary completion estimated March 2027). The Canadian CATCO platform trial lists it among the drugs it may test (NCT04330690, phase 3, last known status active and not recruiting). ChEMBL records a highest clinical phase of 3 (CHEMBL5314991); the only phase 3 record that names the peptide is that platform trial, and its own sponsor's trials are phase 1 and phase 2.

Status as of this entry's last review, November 4, 2026. Approvals, compounding rules and sport rules change; the regulatory tracker is checked more often.

What the studies show

Two indexed papers study this peptide itself: one trial in people and one in mice. Its four registered trials are listed after them. Papers about the receptor DPEP1, and nanoparticles that carry the peptide as a targeting tag, are different subjects and are not cards here.

Multicentre, randomised, double-blind, placebo-controlled, proof of concept study of LSALT peptide as prevention of acute respiratory distress syndrome and acute kidney injury in patients infected with SARS-CoV-2 (COVID-19)

Sixty-one adults in hospital with moderate-to-severe COVID-19 in Canada, Turkey and the United States were randomised to 5 mg of the peptide intravenously each day or placebo for up to 14 days. At 28 days, 28 of the treated patients (93.3%) and 27 on placebo (90.3%) were alive and free of respiratory failure and of continued dialysis, the trial's main question (p=0.86). Fewer treated patients needed intensive breathing support at day 14 (2 of 30 against 6 of 31) without reaching statistical significance, and ventilation-free days were 22.8 against 20.9 (p=0.4). Blood CXCL10 fell further from baseline on the peptide than on placebo (p=0.02). Treatment-emergent adverse events were about as common in both groups. The authors' conclusion is about safety and tolerability; the trial was not built to prove benefit.

Dipeptidase-1 governs renal inflammation during ischemia reperfusion injury

Mice whose kidney blood supply was interrupted and restored developed heavy neutrophil and monocyte infiltration and acute kidney injury. Both were milder in mice lacking dipeptidase-1 and in mice pretreated with DPEP1 blockers, the LSALT peptide among them. The main effect was on neutrophils sticking to the small vessels around the tubules, with less recruitment of inflammatory monocytes. Blocking CD44 reduced neutrophil recruitment as well and added to the DPEP1 effect; blocking ICAM-1 did not. The paper places DPEP1 among the kidney's main leukocyte adhesion receptors and as a target worth testing in acute kidney injury.

Phase 1 in healthy adults, results posted

Fifty-two healthy adults at one Australian site received single intravenous doses from 0.01 mg to 5 mg, or 1, 2.5 or 5 mg once daily for three days, each infused over two hours, against saline. The posted results record no severe, life-threatening or serious adverse event in any group, and a mean half-life of 0.41 hours after a single 5 mg dose and 0.21 hours after three daily doses. Completed March 2020.

Phase 2 in on-pump cardiac surgery

Two hundred and forty patients with kidney-injury risk factors having non-emergency coronary or valve surgery on a bypass pump, randomised to 10 mg or placebo over one hour twice daily for five days, starting at least an hour before anaesthesia. The main measure is acute kidney injury by KDIGO criteria within seven days. Recruiting at nine sites in Canada and Turkey; primary completion is estimated for March 2027, so no result exists yet.
